Annual and Final Performance Reporting Requirements for State Personnel Development

The State Plan required for the State Professional Development Grant application must describe how the SEA will assess, on a regular basis, the extent to which the strategies implemented have been effective in meeting the performance goals described in section 612(a)(15) of the IDEA (effective as of July 1, 2005).
A multi-year award project must submit an annual performance report that provides the most current performance and financial expenditure information.

The deadline to submit reports is May 3, 2013. We recommend grantees read the “Dear Colleague” letter in its entirety, as it contains important information about reporting dates, due dates and performance measures. In addition, it provides information on how to submit your report using e-Reports. Please note, final reports and no-cost extension period reports cannot be submitted through e-Reports at this time. Annual performance reports submitted during a no-cost extension period should be emailed to the grant’s project officer. In addition, a signed cover sheet should be faxed to the project officer or scanned and emailed to the project officer.

Project grantees who are in their final year (e.g. 5th year) and are not applying for a no-cost extension do not need to send in a continuation report. Instead, these grantees must send in their final report within 90 days after the final day of their project.
Project grantees their final (5th year) year, who have remaining funds left to complete activities approved in their application can email their Project Officer a request for a no-cost extension of up to one year. In addition, these grantees must submit a continuation report in the 5th year. Then, 90 days from the final date of their no cost extension they must submit a final report.
Final reports cannot be submitted in the G5 Online Reporting Systems. Grantees must use the 524B form, which is the same form used for annual performance reports (continuation reports) and select “Final Report” on the cover sheet. The directions for completing the final report are part of the continuation report package; however, the final report directions are combined with the annual performance report directions, so the person completing the form should read through the directions carefully.
Where to send the report: Grantees are required to email the entire final report as a PDF, including the original signed cover sheet to their assigned Project Officer. Note: the signed cover sheet can be faxed to the Project Officer or scanned and attached as a PDF to the Project Officer.
Projects may also turn in a final report that contains more extensive information, along with the 524B form. For example, in the past California included a stakeholder report, using graphics to emphasize project results targeting the stakeholder's needs. They also included a descriptive information about the challenges faced in implementing the professional development, supports that assisted the process, how results were experienced, and future plans.

At the time the no-cost extension is requested, about 30 days before the end of the grant, the Project Officer will need a continuation report emailed to them. The Project Officer will also need to know: (1) the amount the grantee has remaining in their budget, (2) the activities the grantee wants to continue to conduct that align with approved objectives, (3) how much of the budget will be used for each activity, and (4) why the grantee was not able to spend the entire budget within 5 years.
